Formally Enhanced Primary Care Plan

If you have a chronic medical condition, you may be eligible for a Chronic Disease Management plan. This will provide you with Physiotherapy, Podiatry, Dietetics or Exercise (Physiology) Therapy consultations under Medicare. Book an appointment with your GP today to discuss your eligibility and any further information.

A Medicare rebate is available for a maximum of 5 services per patient each calendar year. A referral is valid for the stated number of services. If all services are not used during the calendar year in which the patient is referred, the unused services can be used in the next calendar year. However, those services will be counted as part of the 5 maximum rebates for allied health services available to the patient during that calendar year.

Once your doctor has completed the final stage of the Chronic Disease Management plan you are able to book an appointment with us at your earliest convenience. It is important that we have received the plan from your doctor or it is given at the time of your appointment.

There will be no out-of-pocket fees* once you have claimed back your invoice from Medicare. Please ask our reception staff if you need further information about claiming your invoices.
